I hate this hand. Anything could be right.
But, given that this is matchpoints, and despite the fact that partner did not double 2♥, I am leading a low heart. My main goal is to not give up a trick on the opening lead. There is no guarantee that a heart lead won't give up a trick, but I believe that any other lead is more likely to give up a trick.

A heart really does seem normal, I must be missing something. These double inferences are taken too far, it's not like partner needs a solid heart suit for it to be the right lead.

At MPs and maybe even IMPs a heart stands out. A clubs is a 2nd choice but K - small - small is not a good holding to attack from.

Leading a heart seems straightforward on this sequence.
