1NT is a response to the 1♦ opening bid, so it shows the latter type of hand, along with a stopper in hearts. It's not an overcall.

1♦ - (1♥) - 1NT is a response to the 1♦ opening. Generally this will show less than invitational strength and deny 4 spades. The range and minimum quality of any heart stop will depending on agreements.
1♦ - (1♥) - P - 1NT is an advance of the 1♥ overcall. This will typically show around 9-12hcp and deny heart support. The minimum quality of the diamond stop will depend on agreements.
(1♦) - P - (1♥) - 1NT is an overcall. If natural, this will show a strong hand with values in their suits. Many pairs make an agreement to play this overcall artificially though, showing the 2 unbid suits.
