Double by South doesn't promise four - he can be 2335 or such. But it usually will be based on four hearts and north bids acordingly .
North's double asks south to do something intelligent. With a 3424 or such without a spade stopper he will probably pass.

If your question refers to action in fist seat then yes, probably pass rather than open 1D

It's likely there are regionalisms at work as far as what is 'standard'. I'm speaking from North America.
1) I would say that South's double absolutely promises 4 hearts. With 2335, your choices are 3♣ or pass depending on strength (the boundary being about 10-11 hcp), and with 2344, your choices are 3♠ (promising diamond support) or pass (the boundary being about 11-12 hcp).
2) North's double usually shows 1354 distribution (or sometimes 2353) and enough extra strength to make game playable opposite South's promised 10+ hcp. South should have 4 spades including good intermediates (say QT98 or KT9x) to pass. To some extent this depends on vulnerability; when red vs white, one would tend to bid 3N rather than pass.

South is prepared to let opener think he has 4 hearts and enough strength for the 3-level opposite a 12-14 opener. If I were North my Double of 3S would be an 18-19 (too strong to open 1NT) and without 4 hearts.
