I'd lead a middle trump, there is little point on giving declarer diamond ruffs, this could be very wrong if partner was joking with a Jxxxx but I have some confidence in him.

Why? If the auction goes well he can bid 2NT and then hearts and show both suits. And even if the auction doesn't go well, maybe this hand has enough offense to bid hearts at a higher level, or to bid 4NT over 4S emphasizing hearts. Is it really so bad to try to keep clubs in the picture?

Why do you even want to keep clubs in the picture with KQJT9xx of hearts? You always want to declare hearts, and never want partner to correct hearts to clubs.

The only way getting clubs in could help is if it allows partner to bid over their game which seems unlikely. On the downside you might get partner to not let you play 4H, and you don't preempt them very much (look at this hand, it's going to go 1D 4H 4S X, it is not that unlikely that you will push them into something that partner can X IMO by bidding 4H). By not preempting them you also allow them to exchange info at a low level and help their decision. If the plan is really to compete to the 5 level by ourselves anyways, it doesn't seem like we have gained anything by introducing clubs.

Also, why would you bid 4N over 4S to emphasize hearts? You have KQJT9 seventh, just bid 5H!

Maybe we have a slam, or maybe we're supposed to save over their 5-level contract. I know that's not usually our priority when they open but this is a good hand. It seems wrong to just throw up our hands and blast opposite an unpassed partner. And yes, maybe the extra information will help them when it's their hand - but maybe we're getting to the same place in that case no matter what, and by blasting we're throwing away the chance of catching partner with a good hand and the right cards.
