I have heard it many times. The first time that I recall was when I read it in a boys' book (From the 'Kameleon' series) when I was 9 or 10. I wouldn't say it is an extremely common expression, but it pops up once or twice a year.
The difference between my experience and Helene's may be due to the fact that I read boys' books (the gender card). It could obviously also be that I remind people more often of monkeys than Helene does.

It's the monkey that's referred to as being ugly and what can I say, it's generally not a very nice thing to say (especially about people) so it's not a bad thing if you have never heard it.
